How to Use This Page (3 Steps)

  1. Enter Team Information
  2. Add each Team Member using the form
  3. Submit All → then Print

Step 1: Team Information

This information applies to all members you add.

Step 2: Add Team Members

  1. Fill in First Name, Last Name, Role, etc.
  2. Click Add Member — they appear in the preview below.
  3. Repeat for every person on your team.
  4. When done, click Submit All.

Tip: Use Clear All only if you want to remove the entire in-progress team list.
